Please note that you are not required to provide this information to meet with us, but having it available ensures your appointment runs smoothly, allows for better plan comparisons, and helps us give you more accurate advice.

    • List of Your Doctors:

      • Primary Care Physician & Specialists

        • examples: gastro, cardio, podiatry, neuro, endocrine, surgeon, therapies, & don’t forget your optometrist and dentist!

        • (Helpful Tip: The name of the actual MD/Doctor in the practice, not the PA or NP)

    • Your Current Medications:

      • Exact names, precise dosages, and frequency of all current prescriptions

        • (Helpful Tip: copy from the name & dosage directly from the label on the bottle/box)

    • Your Red, White, and Blue Medicare Card:

      • Showing your Part A and Part B effective dates, if available.

    • Current Health Insurance Coverage:

      • Any private health insurance, employer group coverage, union plans, or retiree benefits you are on or may be offered.

    • Any Questions or Concerns:

      • Specific plan features you are looking for or questions you want to make sure we address

    • Income Verifications for the household

      • Such as paystubs, unemployment letters, Social Security statements, retirement/401k withdrawals, pension statements, or self-employment income estimates.

    • Special Enrollment Verification

      • If you have any documents stating your coverage has ended or will end.
